Juan Izquierdo made his debut for Cerro on February 20, 2018, in a Copa Sudamericana match against Sport Rosario. He also made his Uruguayan Primera División debut five days later, coming on as a substitute in a match against Nacional. In his first season, he played 28 games and scored once.

The cause of death was cardiorespiratory arrest related to his cardiac arrhythmia.
